Hospice care

Hospice focuses on the comfort and care for a person who is nearing the end of their life. Hospice workers help improve the quality of the person's life and provide support for their family.

Compassionate hospice services

We give you access to resources that ease your way during a life-limiting illness.

Across Western North Carolina, CarePartners is Mission Health’s post-acute services provider that manages our support resources. We provide medical, physical and emotional support for you and your family during a severely chronic or terminal condition.

End-of-life care and services

The CarePartners hospice teams understand the challenge of living with terminal illnesses. We're committed to offering care that supports you and your family.

Hospice care for end-of-life needs

We offer hospice services through our post-acute care provider, CarePartners. We bring services to your home, nursing home or assisted living residence. Amenities of our program include:

  • Additional medical care, should the anticipated end-of-life expectancy be 6 months or less
  • Bereavement counselors
  • Care plans that reflect the shift in treatment goals from cure to symptom management
  • Chaplains
  • Comfortable, pain-free living space, where you can be surrounded by loved ones
  • Personal aides
  • Physicians, nurse practitioners and medical specialists experienced in compassionate hospice care
  • Social workers
  • Volunteer services

You can access two types of hospice care, including:

  • Respite care — A service providing breaks to caregivers
  • Transitional care — Temporary placement at our inpatient hospice center.

CarePartners Hospice Solace Center

When home hospice or transitional care is no longer an option, our inpatient facility, CarePartners Hospice Solace Center, offers a beautiful, home-like setting. A facility for all ages, it presents tranquil surroundings where one can be at peace. Features include:

  • 27 spacious private rooms with natural woodwork and garden views
  • Chapel with handcrafted stained-glass art
  • Easily accessible natural areas and gardens with a gazebo, a labyrinth and space for visiting or contemplation
  • Family area (great room) with a grand piano and fireplace
  • Fully equipped kitchen and dining room for families

Palliative care

Regardless of your diagnosis, level of treatment or age, we can help you find relief from the symptoms, pain and stress associated with having a serious illness. Team members can assist you in coordinating care across multiple settings, including the hospital and outside community. Palliative care can be provided with curative treatments or in end-of-life care. Our palliative team members are available to discuss your options at (828) 277-4805.

Additional post-acute care services

CarePartners offers programs designed to meet the needs of adults in a variety of home situations, including:

Care for disabled adults

Our Community Alternatives Program for Disabled Adults (CAP-DA) program allows individuals with disabilities to stay at home and receive a variety of community-based services through the Medicaid program which is sponsored through North Carolina's Home and Community Based Waiver Services (HCBS). As unique feature of this program is the ability to choose and hire your caregiver, especially if they live at home with you. CAP-DA serves residents of Buncombe, Henderson, Macon, Jackson and Transylvania Counties. You'll receive care from someone you know and trust, who provides services, such as in-home aides, meal prep, respite care and financial management services.

Private duty and companion services

Our private duty and companion services are in-home offerings that are available without a physician referral. Our nursing aides and companions provide care that helps you stay at home, including personal care assistance, running errands, meal preparation and respite care. Services range from four-hour shifts to 24-hour care.

Program of All-Inclusive Care for the Elderly (PACE)

This program, also known as PACE, provides nursing home-level care for you in your home, including services such as primary care, meal prep, social work counseling and rehabilitative services. If you're over 55 years old and live in Buncombe or Henderson Counties, these services can help you recover in the safety and comfort.

We Honor Veterans Program

If you're a veteran under inpatient care, we offer recognition for you through the We Honor Veterans program. The recognition program honors you in a ceremony at the CarePartners Hospice Solace Center. You and your family receive parchment certificates with gold seals representing your military service branch. We can also place an American flag on your room door. Other accommodations may be made at your or your family’s request.

Bereavement support

At our branch offices, our bereavement coordinators provide services to support families dealing with a loved one's eventual loss. Since loss is handled uniquely by each individual, we offer grief counseling, education and connections with support groups. Volunteer programs

Volunteer services are part of our interdisciplinary approach to care, and our CarePartners Volunteers program provides important services to families.
