Our outpatient clinics see patients with conditions ranging from dizziness to tennis elbow to recovery from a stroke, traumatic injuries to the brain or spine, or amputation.
Do I need a physician referral for my physical therapy appointment?
For many insurance companies, a physician referral is not required for a physical therapy appointment. However, referrals are required for Medicare and Medicaid. We recommend first consulting with your insurance company before calling to schedule an appointment.
What to bring to your first appointment?
- Insurance and/or Medicare/Medicaid card
- Photo ID
- List of current medications/supplements
- Physician order if provided by physician/MD office
- Registration Form
- Medical History Form
- Attendance Policy
- Consent for Treatment
What to wear to your first appointment?
- Dress in loose-fitting, comfortable clothing, and wear shoes with flat, rubber soles.
