Home health services

CarePartners' home health service offers the help you need in the comfort of your home, assisted living facility, independent living apartment, family care home or rest home. The Joint Commission certifies our home health program, recognizing that CarePartners has met the organization's rigorous standards for patient outcomes, safety, and high-quality care.

Our program has provided compassionate, experienced care for over thirty years and now serves Buncombe, Haywood, Henderson, Jackson, Macon, Madison, McDowell, Mitchell, Transylvania and Yancey County.

To get the most benefit from our services, it is best if you have a qualifying health condition and a physician referral.

Home health nurses and aides

If you are recovering from an illness or are chronically ill, terminally ill or disabled, our private duty care and companion service provides support in the comfort of your home. This service does not require a physician referral.

Our multidisciplinary team includes registered nurses, home health aides, social workers, housekeepers and physical, occupational and speech-language therapists. We work according to your needs, so we can complete shifts in as few as four hours, or up to 24 hours. Our aim is to help you retain your independence and continue living at home.

To ensure at-home success and positive outcomes, our specialists help you find financial support, community services and helpful resources when needed. In addition, our experts can help evaluate the safety of your home and work directly with you to create personalized home rehabilitation programs.

Our private duty program is an in-home service available without a physician referral. Our nursing assistants and companion team members provide care that helps you live independently. To that end, we offer personal care assistance, run errands, prepare meals and provide respite care. Services range from 4-hour shifts to 24-hour care. You or your family will pay for this program directly or through a long-term care insurance policy.

Our private duty team has extensive experience working in private care settings and can provide treatment tailored to your condition and age. When everyday tasks like meal preparation or running errands become challenging due to illness, injury or age, our personal and companion team members help you retain independence and continue to live at home.
