We know your time is valuable. That’s why we offer convenient online registration for services such as surgical procedures, mammograms, and diagnostic tests and treatments.

We accept all forms of government insurance from the U.S., Canada and other countries. We also accept Medicare, Medicaid, Civilian Health and Medical Program of the Uniformed Services (CHAMPUS), and workers compensation.

Advance directives are legal documents that allow you to give directions for your future medical care. Advance directives help you protect your right to request or refuse care.

Patient resources

Our post-acute care teams offer you a life with dignity and independence through services in your home, or at our rehabilitation hospital, outpatient clinics or hospice center.

Adult care services

All services, except emergency services, must be authorized by CarePartners PACE, or you may be fully liable for the cost of unauthorized services. Our Program of All-inclusive Care for the Elderly (PACE) allows you to hire a caregiver if you have a qualifying health condition. This service is particularly beneficial if your caregiver lives at home with you. In addition to caregivers, our program provides access to the following:

  • Adult day care
  • Dentistry
  • Emergency services
  • Home care
  • Hospital care
  • Laboratory and X-ray services
  • Meals
  • Medical specialty services
  • Nursing home services
  • Nutritional counseling
  • Occupational therapy
  • Physical therapy
  • Prescription drugs
  • Primary care (including doctor and nursing services)
  • Recreational therapy

Home health services

We provide health, homecare and private duty services, such as nursing, therapy, social work, and personal care. Our experts drive more than a million miles annually, throughout our region, to care for patients like you. To that end, we can deliver services in the comfort of your home, assisted living facility or rest home. We offer 24-hour nursing and can help with many tasks, including shopping, light housekeeping and preparing meals. Most home health services require a physician's referral.

Hospice and palliative care

Our care teams provide physical, emotional and spiritual support if you're managing a terminal illness. Hospice care treats you and your loved ones with respect and compassion, providing the highest levels of comfort and quality of life during their final days. Our palliative team offers relief from the symptoms, pain and stress associated with having a grave health condition. In either care program, we ensure your experience is comfortable, dignified and surrounded by family and friends.

Inpatient physical rehabilitation at our hospital

Our continuum of inpatient care is led by a team of physical medicine and rehabilitation specialists who work with you in our 80-bed hospital, tailoring care to your unique needs. For some, that may mean learning to walk again. For others, it means adjusting to the challenges of a wheelchair or artificial limb, or relearning everyday tasks following a brain injury. Regardless of your health needs, we treat every part of your rehabilitation process as a step closer to a fuller, more independent life.

Outpatient rehabilitation programs

We also provide outpatient therapies to ensure you get the complete care you need, even after you leave our facility. Our programs are designed around your needs, and will help restore your function and well-being.

  • Brain and spine injury rehabilitation
  • Cancer rehabilitation
  • Speech therapy
  • Sports medicine services
  • Stroke rehabilitation
